John Mummert sells a conversion kit I believe as well as Flat-O-Matic. Others on this forum have used both and can probably provide more information.

I have one of Mummerts bell housings and a rebuilt c4. I've been thinking about selling it and going with a t5. Pm me if your interested.

Get the Flat-O-Matic kit. It has everything you need except the trans itself. Good instructions. There is a lot more than just the bellhousing to make the conversion.
